Dilan Ellegala, an accomplished American neurosurgeon, had reached the highest rungs of the American medical establishment but was on the verge of burning out. In search of personal restoration, he risked his career to take a sabbatical at a remote missionary hospital in Haydon, Tanzania.

Dilan began training him to perform brain surgery procedures, giving him the tools to become an agent of change in his own country.
Today, what began as one doctor teaching another has become the NGO Madaktari (Swahili for “doctors”) that has grown into a group that sends hundreds of doctors around the world to serve as mentors and to create a sustainable new model for global health based on Dilan’s “train-forward” philosophy. Captivating, revealing and inspiring. A book about international aid, sex & relationships, and the romance & hardship of a medical career. Definitely a good read.
When nearly 10 years after the development of medicines that turned HIV from a sentence of death to a chronic manageable illness, talk in wealthy nations finally turned to sharing the bounty and making the medicine more widely available on the continent hardest hit by the epidemic, a bizarre and racist argument against doing that had it that Africans couldn’t manage the then more complicated regimen because they didn’t wear wristwatches. Or something like that. In the time since, the fact that people everywhere share a common drive to save their own lives, and those of their neighbors has been demonstrated by progress against that epidemic that has lent itself to talk of ending AIDS globally.
But the condescension behind that original thinking lives on, and remains a major global health threat in its own right.
It was what Dr. Dilan Ellegala, the center of this engrossing nonfiction story that reads like a novel, found when, burned out from the hustle of his education as neurosurgeon, he came to Tanzania, and found visiting western medical students taking front and center seats during clinical presentations at a remote rural hospital.
“So, since this hospital belongs to the Tanzanians, I think it would be better if the visiting students offered them their seats,” he tells the group, as recounted in Send Forth the Healing Sun. “They’re the ones who will be here when we’re gone.”
The visitors surrender their seats, but leave Ellegala wondering what the lasting impact of that, or of their presence there will be. It is the start of an evolution, both personal for Ellagala, who finds a sense of purpose, as well as emotional connectedness, and institutional, in the hospital where he teaches the basics of brain surgery to a medical officer. “See one, do one, teach one,” becomes the lesson from one of his medical school mentors that he applies here, and turns into a movement.
Tony Bartelme, a reporter for the Post and Courier in Charleston South Carolina and the author of Send Forth the Healing Sun met Ellagala when assigned to write a story about the global health program the neurosurgeon launched upon his return to the U.S., according to notes for the book. The writing makes it clear that he enjoyed him, as well as admired him, and that adds to the enjoyment of this intricate book, that explores the nature of education, medicine, aid, and the workings of our most vital organ. In the end, this book makes it clear, it’s all personal.
